How it works

    Take the questionnaire at your own pace. Every question is optional.

  2. Saved as your dated plan

    Your answers are kept as this year's plan, with every earlier year kept in your history.

  3. Guides your Life Rules

    When a rule fires on your monthly Life Statement, your plan shapes what happens next, and you'll see "Your plan says" beside it.

  4. Review each year

    We show you last year's answers and ask, "Still true?" Keep what holds, change what doesn't.

When decisions come up, here's what counts first

  1. Your legal documents, such as your will, trust, power of attorney, health care proxy, and advance directive.
  2. Your current wishes, whenever you're making your own decisions.
  3. Your most recent Future Plan.
  4. Your earlier plans, as a record of how your values have held or changed.

What your Future Plan is, and isn't

  • It is your own record of what matters to you, in your words and on your terms.
  • It is a guide for your Coach, and for the people you choose to share it with.
  • It is not a legal document. It doesn't replace a will, trust, power of attorney, health care proxy, or advance directive, and we'll encourage you to have those in place.
  • It never decides whether you can make your own decisions. That is for you, your physicians, and your legal documents, never a score or a trend.

FAQ

Is my Future Plan legally binding?

No. It's a personal guide. Your legal documents always come first.

What if I change my mind?

Change any answer at any time. Your new answers become a new version, and your earlier plans stay in a history that only you control.

Who can see my plan?

Only you, unless you choose to share it with someone you trust or your advisor.

Can my plan stop me from doing something?

No. Earlier plans guide, they never bind. If something you're doing runs against a firm answer, we'll point it out so you can decide.

Can my health data change whose wishes count?

No. Health signals can prompt a conversation. They never change whose wishes govern.
